This policy explains what Cardinal Point Dental does with the information you share through this website. It covers this website only. Care you receive in our office is covered by our Notice of Privacy Practices.

What We Collect

We collect only what you choose to send us, plus a few technical details your browser shares automatically.

• Contact form: your name, phone number, email address and message, and how you found us.
• Appointment request form: the same, plus your preferred day and time, whether you are a new or current patient, and your insurance if you choose to enter it.
• Technical details: your IP address, browser type and the pages you visit, which the web server records in standard logs.

Please don't put detailed health information or Social Security numbers in a form. Call us on (904) 733-1900 instead.

How We Use It

We use what you send to answer your question, follow up on your appointment request and, for the "how did you find us" answer, to understand which of our outreach people actually see. We do not sell your personal information.

Analytics, Cookies and Embedded Content

This site uses Google Tag Manager to load its measurement tools, and Google Analytics to count visits and see which pages people use. Google Analytics sets cookies on your device and records things like the pages you open, roughly where you are and what kind of device you are on. We use it to understand how the site is used, not to identify you, and we do not use it for advertising. You can opt out withGoogle's browser add-on(Google's policy).

A few features load content from other companies. Those companies may see your IP address and set their own cookies or similar storage when their content loads:

• Google Reviews at the bottom of each page, shown through a widget from Elfsight (Elfsight's policy).
• Videos hosted on YouTube, which load only when you press play (Google's policy).
• The map on the Contact and Request an Appointment pages, from Google Maps (Google's policy).

Who We Share It With

We share information only with the services that help us run the website:

• The form-handling service our web agency operates, which receives what you submit and emails it to our office.
• Our web hosting provider, which serves this site and keeps its server logs.
• The embedded-content providers listed above, when you load their content.

We do not rent or sell your information.

How Long We Keep It

We keep information only as long as we need it to respond to your enquiry and to meet our legal and record-keeping obligations.

Your Choices

Children

This website is not directed at children under 13, and we do not knowingly collect their information through it. A parent or guardian can contact us about a child's care.
